About the Role:

As a Community Care Worker within the Integrated Community Support team, you'll provide essential in-home and community-based services that enable clients to live safely, independently, and with dignity.

This is a field-based role requiring a proactive and flexible approach, excellent time management skills, and the ability to carry out a wide range of basic maintenance tasks with a strong focus on safety and quality service delivery.

Duties & Responsibilities:

• Household tasks including vacuuming, mopping, meal prep, linen changes, and basic cleaning
• Supporting clients with bathing, dressing, toileting, mobility, eating, and prescribed exercise programs
• Providing support to clients and their carers through community engagement and in-home activities
• Supporting client independence while observing and reporting on changes in health or wellbeing
• Work cooperatively within a supportive team environment and contribute to continuous service improvement
• Maintain accurate service records and communicate effectively with supervisors

Requirements:

• Current First Aid Level 2
• Current Victorian Driver’s Licence
• Current Police Check
• Current Working With Children Check
• Covid-19 Double Vaccination Certification
• Relevant trade qualifications or equivalent experience in general home maintenance
• Certificate III or IV in Aged Care, Individual Support or equivalent
• Demonstrated experience working with older people or people with disabilities
• Ability to identify client needs and report changes appropriately
• Strong communication skills, both written and verbal
• Flexibility, teamwork and a client-first mindset
• Basic computer literacy (email, reporting e.g)
